New Patients

Patient wishing to register will be asked to complete a registration form and a health questionnaire. If any repeat prescriptions are required a copy of your repeat prescription slip should also be included with your registration information.

To be eligible to register with us, you must live within our Practice area (see map below) or be staying at an address in the area for 3 months or more. If you will be here for less than that, we can still treat you but you will be registered as a temporary Resident.

Any patient moving out of the practice area will be asked to register with a new practice.

Preference of a Doctor

You will not be registered with any one particular doctor, but you can ask to see a specific doctor, if you so wish—this may mean, however, a longer wait for an appointment.

To Register

1. Download and complete a Registration form via this link (form can be completed online): NHS Inform GP Registration

2. Download and complete a New Patient Questionnaire form: Patient Questionnaire

3. Return the completed forms to the Practice by email to: nhsh.gp55395-admin@nhs.scot or print and post to us if preferred.

4. Please provide proof of identity, either by emailing a copy or bringing it in to the surgery. We do not retain copies of any identification.  Please see the Registration Form for acceptable examples of ID.
